Form 5500 Lookup: Is Your 401(k) Plan a Late Filer?

Every ERISA retirement and welfare plan must file a Form 5500. Miss the deadline and the Department of Labor can assess penalties per day, per filing — and the person who carries that liability is whoever is named as the plan administrator. For most small and mid-sized plans, that is the business owner.

We built a free lookup over the DOL’s public EFAST2 filings so you can see your own plan’s record in about ten seconds: every filing, the original deadline, the Form 5558 extended deadline, and exactly how many days past due each filing landed.

What the lookup shows you

  • Every filing on record — plan year, the date the DOL actually received it, and both deadlines that applied.
  • Days past due, not vague warnings — the same number the DOL uses when it calculates a penalty.
  • Delinquent filer flags — whether a filing went in under the DOL’s Delinquent Filer Voluntary Compliance program.
  • A plain read on the exposure — one late filing is a warning; two makes a pattern.

Who is actually responsible for the filing?

Your advisor recommends. Your recordkeeper processes. Your TPA prepares. None of them sign for the filing. Unless a firm has been appointed in writing as your ERISA §402(a) Named Fiduciary and §3(16) Plan Administrator, the signature — and the liability behind it — is yours.

Admin316 takes that appointment. We don’t just advise the plan. We take responsibility.
